I understand that it is
the intention of the Tulsa Spring Swing(TSS) dance convention to have my
participation in the TSS dance convention recorded on video tape, film,
and/or photographs, and I expressly agree to and grant the unlimited right
and authority to use any recording of my participation in the aforementioned
contest in any media, in perpetuity, in whatever manner and by whatever
means, without obligation to me. Such recordings become the sole property
of the persons or organizations making them.
I understand the physical risks of entering dance
competitions and social dancing, and assume full responsibility for any
injury or personal damages resulting from such activities at TSS dance
convention. I hereby, for myself and my heirs, executors and
administrators, waive and release any and all rights for damage I might have
against the TSS dance convention and its agents for any and all injuries
which may be suffered by me in participation in dance competitions, social
dancing and any traveling to and from the dance convention locations.
As a contestant, I understand that unsportsmanlike
conduct or harassment of the officials will be cause for disqualification or
suspension from current and future TSS dance events and that refusal to
accept an award is considered unsportsmanlike conduct. I certify that I am
18 years of age, or am represented by my Guardian below, if I am under 18
years of age. |
